人氣極旺的名棒球手一郎（一郎 飾）從美國回到日本，引起極大轟動。古畑任三郎（田村正和 飾）和今泉慎太郎（西村雅彥 飾）都是一郎的粉絲，得知向島音吉（小林隆 飾）在一郎的酒店擔任護衛，二人纏著向島幫他們索要簽名，更令他們驚奇的是向島竟然是一郎的哥哥。可是臉上永遠掛著笑臉的向島也有他的煩惱，長久以來他一直被有著黑社會背景的男子郡山繁（今井朋彥 飾）勒索，此番更要想法籌備1000萬日元。走頭無路的向島打算鋌而走險殺掉對方，而與他感情篤深的一郎則要與哥哥共同進行一場完美謀殺案。命案再起，精心策劃和布置的現場，古畑刑警最大的挑戰……
